Non-NHS Services – Forms and Letters requested by Patients
Please ask for advice from reception if you have a form that you think the doctor needs to fill in. You may be asked to leave it at reception and collect it at a later date.
The doctors cannot sign passport applications. A charge will be made for forms that are not covered by the NHS (see list of charges at reception).
Please do not ask a doctor to fill in a form or write a letter during surgery as this causes delays for other patients.
Letters regarding housing will not be written except at the formal request of the housing department or housing association.
Patients may ask the GPs to provide a medical service (e.g. a private examination for insurance/DVLA) or complete an administrative task (e.g. write a report, fill-in a form or write letters for various reasons (letters to Council, Social Workers, Housing, etc.) that is not covered under your NHS entitlements.
For this reason, a fee is payable in advance to the practice in cash. This is non-negotiable and fees may be updated from time to time, in keeping with national guidelines. A current list of fees is available to view in The Surgery.
Payments will be made at the reception desk in cash. A receipt will be issued at the time of payment.
No medical service (e.g. a private examination) or completion of administrative tasks (e.g. write a report or fill-in a form) will be carried out unless payment is received.
- Please leave the form that needs to be completed by the doctor with one of our receptionists with your phone number.
- Reception will liaise with the GP and will inform you telephonically ASAP if you need to be seen in order for the GP to complete the form/write the letter and will let you know how much the fee will be.
- When you make the payment, we will issue a receipt and we will book the appointment for you if necessary (please note this might be a double appointment.)
- Please note that if for any reason you fail to keep the appointment that has been made in connection with this service without notifying the practice within 24hours of the planned appointment time, then you are liable for the full fee for this service and no refund is given.
